23rd Annual UTAH HOSPAC Golf Tournament

The UTAH HOSPAC Golf Tournament is a Shotgun Best Ball Scramble and will begin at 8 a.m. Holes will be assigned at check in beginning at 7:30 a.m. Golf outing registration includes green fee, cart, lunch and prizes. Please indicate on the registration form if you have a preference on the golfers included in your foursome. Otherwise, foursomes will be assigned as registrations are received. Please register by Tuesday, June 6th.

Hole sponsorship and donation opportunities are also available.

Cost: $300/person or $1,200/foursome (includes green fee, cart, prizes & lunch)

Mulligans: Mulligans available online only for $10/each (limit 3 per player or 12 per foursome).

Prizes/Giveaways: You can also indicate on your registration any items you may want to donate for the tournament raffle or goodie bags.
